Our Client is looking for a Care Manager to join their company in Hackensack, NJ.

Salary/Hourly Rate:

$40,000 – $45,000

Position Overview:

As a Care Manager, you will work with families in the community to help them find resources and achieve independence. The Care Manager will assist families with challenges and advocate for them as needed. The Care Manager has to be passionate about helping others in the community.

Responsibilities of the Care Manager:

  • Assist families with available technology in order to support service provision
  • Assist families to access available resources
  • Able to maintain composure in highly stressful situations
  • Builds youth and family satisfaction with the services offered by the organization
  • Ensure that Wraparound Model throughout care management practices and engages families by implementing the Child Family Team process
  • Collaborates with the Family Support Organization to provide caregiver support
  • Empowers youth and their family to utilize informal supports and community resources to successfully transition towards independence
  • Attends meetings and collaborates with involved system partners and completes home visits
  • Ensure all documentation is completed and accurate

Qualifications for the Office Assistant:

  • At least one year experience
  • Maintain a valid driver’s license and own vehicle
  • Experience with crisis intervention a plus
  • Bilingual skills a plus
  • Proof of vaccination is required

Education Requirements:

  • Bachelor’s degree in a human service related field (such as Social Work, Counseling, Clinical Psychology)
