This organization goes above and beyond in their efforts to protect people and save lives and is a phenomenal organization to work for! They are working on a special project to help with the facilitation of the intake of individuals/families that will be immigrating to the U.S. from abroad. The organization will be setting up temporary shelter and medical facilities onsite at various military bases to assist with the intake and evaluation of these families and are looking for Medical Case Managers (RNs) to support the initiative immediately! This is an amazing opportunity to support a community and culture in need and to participate in a U.S. project for social good! Position Overview:

We are looking for several Medical Case Managers (RNs) with excellent verbal and communication skills that can interact with all types and skill levels of individuals. An ideal candidate will be extremely flexible, creative, and self-confident. The Medical Case Manager (RN) will coordinate all onsite medical cases to ensure that appropriate and timely medical services are provided. This task will be achieved through liaison with local health care providers to identify the appropriate care, service and location, thereby supporting the onsite medical team in identifying and managing the healthcare needs of nominated Operation Allies Welcome (OAW) patients and families. The Medical Case Manager (RN) will assure a comprehensive and effective Case Management process in support of OAW patients to ensure functional care pathways are created. Case Pathways must promote effective communication with the medical care team in the identification of individuals requiring Case Management and the utilization of opportunities within the local healthcare system. The Medical Case Manager (RN) will utilize and maintain an approved data collection system in support of the program to demonstrate an effective end to end case management process. This initiative is a 24/7 operation – schedules are on a rotating basis 7 days a week from 7:30AM-5:30pm.

Responsibilities of the Medical Case Manager (RNs):

  • To establish and operate a new case management program that bridges the gap between the complex healthcare needs of the OAW patients
  • Facilitate the capacity and capabilities within the Onsite Medical Teams and the requirement for care within the local health care system
  • Responsible for the tracking, trending, and reporting of managed cases to support the defined reporting for OAW
  • Coordinate patient care throughout the healthcare continuum
  • Collaborate with social services to define patient’s needs and assist in plans for social service care
  • Reassess OAW patient needs based on current evaluation of patient care
  • Serve as an advocate for the OAW patient within the internal and external healthcare system
  • Provide Case manager to track patients and serve as 24/7 POC for local hospitals to provide clinical instructions / discharge instructions
  • Communicate with clinicians’ regarding treatment requirements, such as medications, etc. / Coordinate with evacuation services for patient pick up from local hospital
  • Coordinate and verify insurance eligibility and coverage for specified care with receiving facilities
  • Provide patient education on benefits, services, and other resources / Facilitate communication with patients, providers, and insurance companies to secure successful outcomes
  • Ensure a comprehensive system to evaluate the healthcare needs of the OAW individual and/ or family to facilitate their overall wellness and one that address health issues to enhance the short and long-term outcomes
  • Ensure input from the Quality Assurance and Medical Team/s is collated for reviews and revisions
  • Participate in Interdisciplinary Team Meetings as required with the Onsite Medical Teams Leadership. Maintain accurate case management documentation in accordance with HIPAAA, and other local laws pertaining to confidentiality
  • Responsible for the creation of an OAW Case Management Standard Operating Procedure (SOP)

Qualifications for the Medical Case Manager (RNs):

  • Academic Nurse qualification with current license to practice in the US
  • Case Management Certification a plus
  • ANCC Nurse Case Manager Certification
  • Minimum of 3 years or 2 years fulltime experience in a medical care setting as an RN
  • Completed 30 hours continuing education in the last 3 years
  • Experience with the case management of pediatric and adult patients
  • Understanding of complexities of culturally sensitive healthcare
  • Proficiency in speaking and writing in English/Cultural sensitivity and awareness with the willingness to work with a foreign national patient population
  • BLS, PALS, ACLS and Virginia License REQUIRED

Education Requirements:

  • Associate’s or Bachelor’s degree from approved nursing program is required

Benefits:

  • Mileage reimbursement of .585 cents per mile for candidates traveling over 50 miles to base
