Our client is a nonprofit organization committed to supporting children and mothers by delivering resources, education, and care that strengthen families and foster long-term well-being. They are currently seeking a dedicated Case Manager to join their team and support maternal and child health initiatives throughout the community.
Salary/Hourly Rate:
$18/hr
Position Overview:
The Case Manager is a frontline public health professional who builds trusted relationships within the community, connecting women and families to healthcare, social services, and maternal and child health programs. This role delivers outreach, education, and comprehensive case management through both virtual and in-home visits. The Case Manager will serve as an advocate and resource for clients, helping ensure equitable access to healthcare, social services, and community-based support programs while promoting positive maternal and child health outcomes.
Responsibilities of the Case Manager:
- Conduct community outreach and deliver education through both virtual and in-home visits to support women and families.
- Provide comprehensive case management services, including enrollment support, referrals, and care coordination.
- Connect clients with healthcare providers, maternal and child health programs, and local social service resources.
- Offer informal counseling, advocacy, and social support to help ensure equitable and timely access to care.
- Assist with program outreach initiatives, including recruitment efforts, workshops, and community resource mapping.
Required Experience/Skills for the Case Manager:
- Bilingual in English and Spanish.
- Experience in maternal and child health, public health, community health, or a related field.
- Strong understanding of healthcare and social service systems in Mercer County and New Jersey.
- Demonstrated ability to provide case management, client advocacy, and care coordination for diverse populations.
- Excellent communication and interpersonal skills.
- Comfortable conducting both virtual and in-home visits and engaging families in community-based environments.
- Valid driver’s license and ability to travel regularly throughout Mercer County.
Preferred Experience/Skills for the Case Manager:
- Experience working with maternal and child health programs.
- Background supporting underserved or diverse community populations.
- Experience conducting community outreach, education, and advocacy initiatives.
Education Requirements:
- Bachelor’s degree in Social Work preferred but not required.
