Our client is a nonprofit dedicated to supporting children and mothers by providing resources, education, and care that strengthen families and promote long-term well-being. They are seeking a Case Manager to join their team.
Salary/Hourly Rate:
$18/hr
Position Overview:
The Case Manager is a frontline public health professional who builds trusted relationships within the community to connect women and families to healthcare, social services, and maternal and child health programs. This role provides outreach, education, and comprehensive case management through virtual and in-home visits.
Responsibilities of the Case Manager:
- Conduct community outreach and provide education through virtual and in-home visits to support women and families.
- Deliver comprehensive case management, including enrollment assistance, referrals, and care coordination.
- Connect clients to healthcare, maternal and child health programs, and local social service resources.
- Provide informal counseling, advocacy, and social support to promote equitable and timely access to care.
- Support program outreach efforts by assisting with recruitment, workshops, and community resource mapping.
Required Experience/Skills for the Case Manager:
- Bilingual in English and Spanish.
- Experience working in maternal and child health, public health, community health, or a related field.
- Strong knowledge of Mercer County and New Jersey healthcare and social service systems.
- Ability to provide case management, client advocacy, and care coordination with diverse populations.
- Excellent communication and interpersonal skills.
- Comfortable conducting virtual and in-home visits and engaging families in community-based settings.
- Valid driver’s license with the ability to travel daily within Mercer County.
Education Requirements:
- Bachelor’s degree in Social Work preferred but not required.
