Our client, a value-based cancer care company, is seeking compassionate Remote Oncology Nurse Navigators to join their team!
Salary/Hourly Rate:
$39.90/hr
Position Overview:
As Remote Oncology Nurse Navigators, you will be on the front lines serving members who have been diagnosed with cancer. This role reports to the RN Team Lead. In this position, you will conduct clinical assessments, monitor for changes in health, coordinate care, including transitions, and educate members and caregivers about their diagnosis and treatment over the phone to support our members as they move through the oncology care continuum. You will demonstrate a strong clinical focus, supporting the need for culturally competent care. You will also assist with other administrative projects as needed.
Responsibilities of the Remote Oncology Nurse Navigators:
- Establish trusting relationships with members and their care network.
- Assist with screening, survivorship, and end-of-life care.
- Take charge of tasks that include symptom management, nutritional support, discharge planning, and provider referrals.
- They will also handle urgent clinical escalations and provide clinical consult support.
- Assist members with care coordination and care management following admissions.
- Coordinate discharge plans with hospital case managers and follow-up care with providers.
- Collaborate with non-medical team members to address the factors affecting patients’ health.
- Carry out phone-based evaluations, encompassing pain management and medication reviews.
Required Experience/Skills for the Remote Oncology Nurse Navigators:
- The Remote Oncology Nurse Navigators will ideally possess an OCN.
- Must have a current Compact license.
- Must have availability to work Saturday + 4 Weekdays 8:30 am – 5:00 pm EST.
- Compact nursing license.
- Licensure in either NY State or IL.
- 5+ years of nursing experience with 3 years of Oncology Nursing or Case Management.
Education Requirements:
- Bachelor of Science in Nursing.
